All Parent Coaches and Assistant Coaches (Recreation) and Parent Managers and Assistant Parent Managers (Travel) must undergo a fingerprint background check.  The instructions are on the Chatham Recreation website.  There are not exceptions to this rule and no one will be allowed to coach without undergoing the fingerprint background check and obtaining the approval of Chatham Recreation.

If you have never been through a fingerprint background check you must follow the instructions for an "Initial Background Check."  If you have undergone an Initial Background Check and are up for renewal you must follow the instructions for a "Supplemental Background Check."

Initial Background Check - Fingerprint Check:
  • All categories noted within I-A (Coaches, assistant coaches, trainers, staff assistants or any individual that may have supervisory responsibility for underage children that are involved with any of the club or recreation sports programs) who have not had a fingerprint background check with Chatham Recreation will need to complete this process.
  • Prior to ANY coach, etc. receiving a FINGERPRINT check form/code from Chatham Recreation:coaches, assistants, etc. will need to complete the Criminal History Fingerprint and Background Name Check Consent And Release of Liability Form (Appendix 1). This form will be available on the Chatham Recreation website at chathamrecreation.org under Fingerprint Backgrounds.
  • This form after completion, is to be returned to Chatham Recreation at 54 Fairmount Ave. The form must be the original notarized form (No copies or emailed versions).
  • Upon receipt of this form by Chatham Recreation, the individual will receive the New Jersey Universal Fingerprint Form to schedule a fingerprint appointment. 
  • Upon completion of the fingerprints, the coded form is to be returned to Chatham Recreation.
  • Fingerprint results will be sent directly from the state to Chatham Recreation. They are usually received within about 7 business days.
 
Supplemental Background CheckRenewal of those having the original Fingerprint Check
  • All categories noted within I-A (Coaches, assistant coaches, trainers, staff assistants or any individual that may have supervisory responsibility for underage children that are involved with any of the club or recreation sports programs) who have had a fingerprint background check with Chatham Recreation will need to complete this process.
  • Coaches, assistants, etc. will need to complete the Criminal History Fingerprint and Background Name Check Consent And Release of Liability Form (Appendix 1).  This form is available on the Chatham Recreation website at chathamrecreation.org under Fingerprint Backgrounds - Consent/Release of Liability. Go to that page and click on the form to print.
  • This form after completion, is to be returned to Chatham Recreation at 54 Fairmount Ave. The form must be the original notarized form (No copies or emailed versions).
  • Coaches, assistants, etc. will then need to complete the New Jersey State Police Form (Appendix 3). This form will be available in the Chatham Recreation Office, 54 Fairmount, by regular mail, can be distributed at meetings and/or at “scheduled renewal days”. This form can NOT be submitted for processing without the notarized Consent And Release of Liability Form.


We thank you in advance for your cooperation of this request. Remember, this is required in order to be approved as a volunteer coach.

Additional Details

Policy:
The Chatham Borough Council and Chatham Township Committee have adopted resolutions that require the adult members of the Joint Recreation Program and the members of the Club Sport Programs to submit to an initial background check and supplemental background checks as outlined in this policy.  

 

Procedures:

I.          Initial Background check
 
A.    Coaches, assistant coaches, trainers, staff assistants or any individual that may have supervisory responsibility for underage children that are involved with any of the club or recreation sports programs must comply with the Joint Recreation background check procedures.
 
1.     Any individual that does not comply with the Joint Recreation background check procedures is not authorized and shall not be assigned to or involved with any of the sanctioned club or Joint Recreation program activities.
 
B.    The initial background check shall consist of the following:
 
1.     Signed waiver by the applicant authorizing the Joint Recreation Department to share with the club president, all information, including reports from the New Jersey State Police and other law enforcement agencies. (Appendix #1)
 
a.     The applicant must submit the signed waiver to the Recreation Department before the submission of his/her fingerprints.
 
2.     The applicant shall have his/her fingerprints taken for submission to the New Jersey State Police and Federal Bureau of Investigation.
 
3.     The applicant shall complete the fingerprint applicant registration form and schedule an appointment with the approved agency that is listed on the fingerprint applicant registration form. (Appendix#2)
 
a.     The applicant shall provide a copy of the fingerprint applicant registration form to the Recreation Department.

 

II.             Supplemental Background  Checks

 
A.    Supplemental background checks shall be completed no more than four years from the date of the initial background check and thereafter within four year intervals.
 
1.     The supplemental background check shall be a name check submitted to the New Jersey State Police.
 
a.     New Jersey State Police form SPI212B is to be completed by the applicant and submitted to the Joint Recreation Department for processing. (Appendix #3)
 
B.    Supplemental background checks shall consist of the following:
 
1.     Signed waiver to authorize the Joint Recreation Department to share with the club president all information, including reports from the New Jersey State Police and other law enforcement agencies. (Appendix #1)
 
2.     The applicant shall complete New Jersey State Police Form SPI212B and submit the form to the Recreation Department for processing. (Appendix #3)

 

III.            Initial and Supplemental Background Check Expenses

 
A.    The initial and supplemental background expenses will be paid from the Joint Recreation Trust Fund.
 
1.     The Recreation Department is responsible to verify the initial and supplemental background check expenses and prepare purchase requisitions to reimburse the applicant or directly pay the New Jersey State Police for the expenses associated with the initial and supplemental background checks.

 
      B.    The cost for the initial and supplemental background checks shall be included and made part of the field maintenance fee.
